Please keep a copy of your sales receipt and this rebate form for your records. Selected Electrolux models listed purchased
between October 8, 2009 and November 29, 2009 are eligible for the Amazing Savings Laundry Event rebate. No substitution of other
models is allowed. This promotional offer may not be used in conjunction with other Electrolux offers. Incomplete submissions will be
returned to the sender for re-submission with complete documentation. Claim MUST be postmarked no later than January 28, 2010.
No re-submissions will be accepted after February 27, 2010. Delivery must be completed by January 28, 2010. The consumer is
responsible for appropriate state sales tax. Allow up to 12 weeks for delivery. Inquiries about the status of a claim can be made in writing
and mailed to Amazing Savings Laundry Event Rebate, Campaign Code 09-67597, P.O. Box 540055, El Paso, TX 88554-0055; by calling
1-866-727-0582; or by going to electroluxrebates.com. (Please retain this information for future reference.) Some restrictions may apply.
Offer applies to retail customer purchasing through participating retail dealers only. The Amazing Savings Laundry Event rebate is
applicable to individual sales only. Multiple sales to apartments, condominiums, subdivisions, wholesale sales, etc., do not qualify. This claim
form is non-transferable. Photocopies of this rebate form will be accepted. Completed materials will not be returned. Owner warranty
registration card, if mailed with this claim form, will be submitted to a service company on your behalf. Not responsible for lost, late or
