November 1, 2009 and January 2, 2010 are eligible for this rebate. No substitution of other models is allowed. Incomplete
submissions will be held and the submitter notified of missing documentation. Claim MUST be postmarked no later than
January 16, 2010. No re-submissions will be accepted after January 30, 2010. If you purchased your appliance(s) as part of a
new home construction or remodeling project, you may submit your claim upon delivery of your appliances (postmarked by
February 27, 2010), provided the receipt was originally generated and dated within the program period. MUST check box at

sales only. Multiple sales to apartments, condominiums, subdivisions, wholesale sales, etc. do not qualify. Claim paid on ONE
model per product category. Checks not cashed within 90 days of date on face of check will not be re-issued. This claim form
is nontransferable. Photocopies will be accepted. Completed materials will not be returned. Owner warranty registration card
if mailed with this claim form will be submitted to service company on your behalf. Not responsible for lost, late or misdirected
